In addition to general medical consultations we are pleased to be able to provide specialist clinics and services covering the following areas:

• Asthma – If you have been advised by the surgery to submit an annual review of your asthma symptoms, please complete our Asthma triage
• Chronic obstructive airways disease
• Coronary heart disease – If you have been advised by the surgery to complete an annual heart disease review, please complete our Annual Coronary Heart Disease Review form
• Cervical screening
• Contraceptive services – If you have been advised by the surgery to submit a contraceptive pill review, please complete our Contraceptive Pill Review form
• Child health surveillance
• Diabetes – If you have been advised by the practice to complete a diabetes review, please complete our Diabetes Review form

We also have access to physiotherapists, chiropodists, dieticians, counsellors and psychologists. Their services can be assessed by a referral from your GP.
